Good billing operations make the next action clear and repeatable. A queue should tell a team what needs attention, who owns it and when to act.
A QUESTION FROM THE WORK
Is a clean claim the same as a paid claim?
Quick answer
No. In the HFMA clean-claim measure, the question is whether a claim passes processing-tool edits without manual intervention. That is a different stage from the payer’s final payment decision.
Illustrative example
Illustrative workflow: a claim passes the billing tool’s edits, then the payer requests additional information during adjudication. The first result describes edit performance, not a payment guarantee.
Common mistake
Using first-pass tool performance, payer acceptance and payment rate as interchangeable labels.
Reporting / payer considerations
Define which edits and processing stage your report measures. Keep rejection and adjudication outcomes alongside—not inside—a differently defined metric.

Organize around the work that can move
Separate claims ready to submit from accounts waiting for a correction, documentation, a payer response or a practice decision. Give each queue a clear entry condition and a clear exit condition. A long list of open accounts is an inventory; it is not yet a plan.
Build quality into the routine
Review a small, deliberate sample of completed work. Look at the reason for the action, the supporting information, the note and the next step. Share the lesson with the team and check whether the same issue appears again.
Give the weekly meeting a decision
Bring unresolved dependencies, repeated errors and exceptions that need leadership input. End with an owner and a due date for each decision. The meeting is useful when the next week’s work becomes clearer.
Questions to take into the work
- Can a new team member explain what “complete” means?
- Which exceptions keep returning to the same queue?
- What did the last review change?
